New Student Enrollment
Complete your form online then make an appointment to come to the building
to finalize your registration.
Follow these easy steps for enrollment.
1. Follow the link below to complete all forms prior to arrival.
2. Make an appointment with the registrar to finalize registration
3. Bring All Required Documents
4. Complete your Final Forms online, which includes important
Emergency Medical and Transportation information.
Complete form and submit. If you stop before form is complete
you cannot go back, you must start over.
Call our Registrar, Genda Pope, at 937/444-2529 ext. 22001 for
between 8am and noon for an appointment.
Please note the office is closed the month of July, but you can
complete your forms and call for an appointment in August.
Bring Documents Required for Enrollment:
-Student Birth Certificate
-Student Social Security Card
-Parent State ID or Drivers License
-Proof of Residence (current Electric or Utility Bill or Rental Agreement)
If you are living with someone else and do not have access to a Utility
bill in your name please download and complete the Affidavit II below
-Custody Papers (if applicable)
-IEP or WEP (if applicable)
Note: A working e-mail address for parent or guardian will be required
to complete all Emergency Medical and Transportation forms.
