3. Bring Completed Forms and Required Documents to Appointment
4. Complete your Final Forms online, which includes important
Emergency Medical and Transportation information.
New Student Enrollment (1st - 12th Grade)
1. Forms to Download
If unable to download, packets can be picked-up at the District Office.
2. Call our Registrar, Genda Pope, at 937/444-2529 ext. 22001 for
an appointment. Morning appointments can typically be
registered and begin school the next day.
3. Bring Documents Required for Enrollment:
-Student Birth Certificate
-Student Social Security Card
-Parent State ID or Drivers License
-Proof of Residence (current Electric or Utility Bill or Rental Agreement)
If you are living with someone else and do not have access to a Utility
bill in your name please download and complete the Affidavit II below
-Custody Papers (if applicable)
-IEP or WEP (if applicable)
Note: A working e-mail address for parent or guardian will be required
to complete all Emergency Medical and Transportation forms.
